PCDH
The information about the dispensing of medicines is uploaded in a standardized manner in a “Pharmaceutical Care Data Hub” (PCDH) managed by the non-profit organization FarmaFlux.

- Incorporates the referral register (see below)
- It stores types of data subject to exchange in the context of pharmaceutical care
It has a fine grained authorization mechanism for data management
Referral Register
The referral register is part of the Pharmaceutical Care Data Hub and is a patient-centric registry that contains references to separate storages of patient data outside of the Pharmaceutical Care Data Hub. It adheres to the HUB-METAHUB approach:
- The register informs the eHealth Meta-HUB if patient data is stored in the Pharmaceutical Care Data Hub.
The register requests the eHealth Meta-HUB if extra information is available in the other hubs
